VITAL RECORDS INFORMATION

The following vital records services are provided by the City of Mansfield:

  • Certified copies of birth and death certificates
  • Permits for burials and cremations to funeral homes

BIRTH CERTIFICATES

Birth records are confidential for 75 years and certified copies can be issued to qualified applicants (self, parent, grandparent, stepparent, spouse, sibling, child, legal representative, guardian, military recruiter). The Vital Records Section provides certified copies of birth certificates to anyone born in the city of Mansfield from 1989 to the present.

Birth records prior to 1989 can be obtained by calling the Tarrant County Clerk's Office at 817-884-1195. All requests for birth certificates must be submitted on an application form. Or send us a Request so that we can mail you the appropriate forms. The application, check or money order, photocopy of ID and a self-addressed stamped envelope should be mailed to 1200 East Broad Street, Mansfield, Texas 76063. The check or money order should be made payable to the "City of Mansfield". Requests will be processed and mailed out the next working day. For additional information call (817) 276-4203.

Fees (Effective December 1, 2005)

Each certified copy of a certificate of birth: $23.00


DEATH CERTIFICATES

Death records are confidential for 25 years and certified copies can be issued to qualified applicants (spouse, parent, grandparent, stepparent, sibling, child, grandchild, legal representative/guardian, informant, funeral home).

The Vital Records Section provides certified copies of death certificates for deaths that occurred within the city limits of Mansfield since June, 1987. Death records prior to June, 1987 can be obtained by contacting the Texas Vital Statistics at 1100 West 49th Street, Austin, TX 78756-3199. The toll-free phone number is (888) 963-7111. The fax number is (512) 458-7506.

All requests for death certificates must be submitted on an application form. Or send us a Request so that we can mail you the appropriate forms. Fees (Effective December 1, 2005)

Certificate of Death: $21.00 Note: The fee for the first certified copy of a certificate of death is $21 and the fee for each additional copy of the same certificate requested at the same time is $4.

When requesting a certified copy in person, the requesting individual must present a picture ID. If you fax or phone in your request, payment must be made prior to processing the application.


NON-RESIDENT RECORDS

To obtain certified copies of death certificates from cities other than Mansfield, you may contact the city where the person died. If the person died in Texas, you may contact the Texas Vital Statistics at 1100 West 49th Street, Austin, TX 78756-3199. The toll-free phone number is (888) 963-7111.
